Major Wozniak was a member of the 11th Tactical Reconnaissance Squadron at Udorn Air Base, Thailand. On January 17, 1967, he was weapons systems officer a McDonnell Phantom II Reconnaissance Fighter (RF-4C) on an unarmed reconnaissance mission over Than Hoa Province, North Vietnam, when his aircraft was shot down. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
