Notes
Colonel Tyler was a member of the 390th Tactical Fighter Squadron. On October 24, 1968, he was the pilot of a McDonnell Douglas Phantom II Fighter (F-4D) on a mission over North Vietnam, when his aircraft crashed. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
