Chief Warrant Officer, Third Class Smith was a member of Company A, 9th Aviation Battalion, 9th Infantry Division. On September 27, 1968, he was the aircraft commander of a Hughes Loach Observation Helicopter (OH-6A) on a command and control mission near My Tho, Kien Hoa Province, South Vietnam when the aircraft was hit by enemy fire and crashed into the My Tho River. His remains were recovered on May 5, 1999 and identified on August 23, 2000. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
