Colonel Smith was a member of Headquarters and Maintenance Squadron 11, Marine Air Group 11, 1st Marine Air Wing. On August 19, 1969, he was flying in a McDonnell Douglas Phantom II Fighter (F-4B) which departed Danang, South Vietnam to escort a photo reconnaissance mission just north of the Demilitarized Zone. After a successful run 5 miles east of Vinh Linh, Quang Binh Province, North Vietnam his aircraft failed to meet the escorted aircraft and he was declared missing. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
