Notes
Major Smith was a member of the 29th Tactical Airlift Squadron. On May 30, 1968, he was the pilot of a Cessna Skymaster Observation Aircraft (O-2A) acting as a forward air controller about 15 miles east of Chavane, Saravane Province, Laos, when his aircraft was shot down. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
