Chief Warrant Officer, Second Class O'Neill was a member of Headquarters, 37th Signal Battalion, 1st Signal Brigade. On April 3, 1972, he was the pilot of a Bell Iroquois Utility Helicopter (UH-1H) on a resupply mission to signal units around Quang Tri City, South Vietnam that was shot down. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
