Specialist, Sixth Class O'Hara was a member of 282nd Assault Helicopter Company, 212nd Aviation Battalion, 17th Aviation Group, 1st Aviation Brigade. On February 6, 1969, he was the crew chief of a Bell Iroquois Utility Helicopter (UH-1H) on a re-supply mission to Quang Tri Province, South Vietnam. Bad weather caused the aircraft to return to the Vandergrift Landing Zone, but contact was lost. His remains were recovered on August 27, 1996 and identified on October 12, 2000. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
