Major Mitchell was a member of the 1st Air Commando Squadron. On January 14, 1964, he was the pilot of a Douglas Invader Bomber (B-26B) on a napalm drop about 30 miles northeast of Bien Hoa and 5 miles north of Cau Tri An, Dong Nai Province, South Vietnam. After his first drop the aircraft was hit by ground fire, crashed and burned. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
