Warrant Officer, First Class McIntosh was a member of Company A, 2nd Battalion, 17th Cavalry Regiment, 101st Airborne Division. On November 24, 1970, he was an observer in a Hughes Cayuse Observation Helicopter on an armed reconnaissance mission with two Cobra gunships south of Khe Sanh, South Vietnam. After a Cobra strike, the Observation Helicopter flew in to view the damage, was hit by ground fire, crashed and burned. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
