Major Horsky was a member of the 309th Air Commando Squadron. On December 11, 1965, he was the pilot of a Fairchild Provider Cargo Aircraft (C-123K) on a mission over Phu Yen, South Vietnam, when his aircraft crashed. His remains were recovered on June 16, 1974 and identified on February 27, 1979. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
