Notes
Lieutenant Colonel Holmes was a member of the 22nd Tactical Air Support Squadron. On March 15, 1966, he was the pilot of a Cessna Bird Dog Observation Aircraft (O-1G) as a forward air observer over a truck park on the Ho Chi Minh Trail in Laos when his aircraft was struck by anti-aircraft fire and crashed on the east side of the Se Nam Kok River 300 meters from Ban Keng Khan Kao. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
