Notes
First Lieutenant Hale was a member of Troop B, 2nd Squadron, 17th Cavalry Regiment, 101st Airborne Division. On March 8, 1971, he was the pilot of a Hughes Cayuse Observation Helicopter (OH-6A) on an armed reconnaissance with an AH-1G Cobra gunship. He dropped incinerate grenades to start a fire on a hill top. The aircraft returned to the site to see if the fires had started when it received ground fire. The aircraft crashed and exploded upon impact. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
