Notes
Sergeant Frazier was a member of 191st Assault Helicopter Company, 214th Aviation Group, 1st Aviation Brigade. On September 3, 1968, he was a crewman on a Bell Huey Utility Helicopter (UH-1C) flying over South Vietnam when the aircraft crashed and exploded. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
