Notes
Major Crew was a member of the 389th Tactical Fighter Wing, Da Nang Air Base, South Vietnam. On November 10, 1967, he was the bombardier/navigator of a McDonnell Douglas Phantom II Fighter (F-4C) on a mission to Dong Ha, North Vietnam. During the bombing dive radio contact was lost. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
