Captain Cook was a member of the 21st Tactical Air Support Squadron. On October 21, 1969, he was the pilot of a Cessna Skymaster Observation Aircraft (O-2A) on a mission over the juncture of Tuyen Duc, Ninh Thuan and Khanh Provinces in South Vietnam, when his aircraft was shot down by hostile fire. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
