First Lieutenant Cook was a member of the 555th Tactical Fighter Squadron. On September 21, 1972, he was the bombardier/navigator of a McDonnell Douglas Phantom II Fighter (F-4D) on a mission over the Plain of Jars, where his aircraft was hit by hostile fire and crashed. His remains were recovered on June 3, 1994 and identified on September 29, 1995. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
