Major Carroll was a member of the 56th Special Operations Wing, Udorn Airfield, Thailand. On November 7, 1972, he was the pilot of a Cessna Bird Dog Observation Aircraft (O-1) on a mission over the Plain of Jars, Xiangkhoang Province, Laos. About 5 miles southwest of ban Na Mai, his aircraft was struck by hostile fire and crashed. His remains were recovered on April 9, 2007 and identified on April 30, 2007. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
