Staff Sergeant Caldwell was a member of the Quartermaster School, Training Advance Detachment, Training Directorate, Military Assistance Command (MACV). On December 14, 1971, he was a passenger in a Beech Ute Utility Aircraft (U-21A) on an administrative flight from Phu Bai to Da Nang, South Vietnam when the aircraft caught fire and crashed. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
