Notes
Major Booth was a member of the 497th Tactical Fighter Squadron. On June 23, 1968, he was the navigator of a McDonnell Douglas Phantom II Fighter (F-4D) on a night mission to bomb targets in the mountains in Quang Binh Province, North Vietnam near the Laotian border. As the aircraft made its bombing run it crashed and exploded on the side on a mountain. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
