Major Avolese was a member of the 2nd Bomber Squadron. On July 7, 1967, he was a member of the crew of a Boeing Stratofortress Bomber (B-52D) 20 miles offshore over the South China Sea, when it collided with another B-52D bomber. His remains were not recovered.

Through the work of the Defense POW/MIA Accounting Agency, the remains of Major Paul Andrew Avolese were accounted for in 2020. His name is permanently inscribed on the “Courts of the Missing” at the Honolulu Memorial.
