Captain Van Dyke was a member of the 480th Tactical Fighter Squadron. On September 11, 1968, he was the pilot of a McDonnell Douglas Phantom II Fighter (F-4D) on a mission 5 miles south of Quang Khe, Quang Binh Province, North Vietnam, when his aircraft was shot down. He was taken prisoner and died from injuries. His remains were recovered on July 7, 1981 and identified on July 20, 1981. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
