Notes
Captain Smith was a member of the 390th Tactical Fighter Squadron. On January 17, 1969, he was the weapons systems officer of a McDonnell Douglas Phantom II Fighter (F-4D) on a mission 10 miles northeast of Sepone, Savannakhet Province, Laos where his aircraft was shot down. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
