Airman First Class Miyazaki was a member of 12th Fighter Commando Squadron. On January 31, 1967, he was a crew member of a Fairchild Provider Cargo Aircraft (UC-123B) on a defoliation mission about 5 miles south-southwest of Sepone, Savannakhet Province, Laos when the aircraft was hit by hostile fire and crashed. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
