Notes
Major Johnson was a member of the 355th Tactical Fighter Squadron. On October 27, 1966, he was the pilot of a Thunderchief Fighter (F-105D) on a bombing run near Quang Khe, North Vietnam, when his aircraft burst into flames and break up in the air. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
