Major Jefferson was a member of the 390th Tactical Fighter Squadron. On May 12, 1967, he was the bombardier/navigator of a McDonnell Douglas Phantom II Fighter (F-4C) on a mission near the border of Ha Tay and Hoa Binh Provinces, North Vietnam, when his aircraft was hit by hostile fire and crashed. His remains were recovered on July 14, 1999 and identified on June 5, 2000. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
